Freeform Text, Numbers, Date, Time

Email addresses, zip codes, and any form of ‘free response’ can be collected using the Freeform Text or Numbers form elements. By selecting ‘multiple lines’ in the Freeform Text elements, users can click return and respond in multiple sentences. 

Date fields can be used to collect birthdays, date of signature, date of sample collection, and the like. 

Time fields are often used to confirm when a user would like to set their notifications. 

If using the time field for sleep research (e.g. bedtime), consider adding a wearable for comprehensive, objective data. Information on licensing Chloe wearable integration is available by contacting accounts@peoplescience.health.
