Study Product Feedback Form

When requesting product feedback via forms it is helpful to ask clear and specific (e.g. How was the smell of the study product?), open-ended questions. This approach allows participants to provide detailed and nuanced responses, offering deeper insights into their experiences with the product. 

In addition, monitoring for any reports of side effects or adverse events is critical. Using the send email action within a form will automatically notify the appropriate study team members via email. Always defer to the guidelines of relevant regulatory bodies when running clinical trials.
