Multi-Submission Forms

In the study template, standard forms can be configured to “Allow multiple form submissions”. After setting the date that the form becomes available, the multiple form submissions enabled form will be available until the study end date.

Multi-submission forms can be submitted more than once per day. This may be useful in the case of a symptom tracker, adverse event tracker, or food log. The Notebook Form Element can make these submissions visible to the participant.

Symptom tracking

To set up time based symptom tracking, schedule a recurring form in the Study Template.

Adverse event tracking

As an alternative to weekly tracking, by having an always available form, participants can log symptoms and adverse events. Adding the Send Email Action to the form can notify select study staff when a participant endorses an adverse event.
